·          Support to move on to independent living

·          Up to 3 tenants in house at one time

·          24 hour on-call support

·          Exit plan
Who is eligible

·          Eligible for DSQ registration

·          Has unsuitable accommodation

·          On the waiting list for Community Housing

·          At least 18 years of age & have Australian permanent residency

·          Has a primary diagnosis of mental illness

·          Wants to receive support for setting & achieving personal goals

·          Wants to live independently

 
Referral Process

·          Referred to anam cara

·          Register with Disability Services Queensland (DSQ) http://www.disability.qld.gov.au

·          Eligibility assessment

·          Visit Blackwattle House or Oxford House

·          Accepted on programme

·          Support plan developed

·          Tenancy Agreement signed

·          Move to Transitional House
